During your first visit a thorough examination will be completed. This exam will include any necessary x-rays, as well as other aids which may be needed to make an accurate diagnosis of the condition of your mouth, teeth, and gums. Your dental condition can then be adequately determined and a suitable treatment plan can then be discussed with you. In most instances, a cleaning and polish is also performed at this visit.

As a new patient, a dental record release form ensures that your health information is shared legally and ethically, while also protecting the privacy of your dental records. The form also helps maintain continuity of care by providing us with your complete dental history.
